We respect your right to privacy and confidentiality. By agreeing to the Terms & Conditions you are acknowledging that, in completing and submitting AHP’s Pre-Assessment Questionnaire, all of your responses, including any uploaded photos, may be shared with your Employer.

You will be asked to provide some personal details and information related to your employment and working from home arrangement, including photos of your workspace. You will also be asked some questions about your physical health. You are not obligated to provide medical in-confidence information, should you not wish to do so.

We strongly advise that you remove any confidential or personal information from your workspace prior to completing this task and ensure that there are no people in any photos you choose to upload, including yourself.

Neither you nor your Employer are under any obligation to comply with any of the advice provided or recommendations made by Active Health Partners.
