Active Health Partners (AHP) respects your privacy and confidentiality. Information you provide through this platform is securely stored using encrypted, cloud-based systems.

This platform is provided by AHP to your Employer. By proceeding, you acknowledge that you are using this system as an employee or representative of your Employer.

1. Data Collection & Use
In completing and submitting the Working From Home Checklist & WHS Training form, you acknowledge and agree that:

  • All responses, including any uploaded images, will be shared with your Employer

  • Your Employer is the data owner and is responsible for how this information is used and managed

  • AHP acts solely as a service provider and data processor on behalf of your Employer

You may be asked to provide personal information, including details about your work environment and aspects of your physical and mental wellbeing. You are not required to disclose personal or sensitive information that you are not comfortable sharing with your Employer.

2. Use of De-identified Data
AHP may use aggregated and de-identified data derived from this platform for the purposes of:

  • Improving the platform and user experience

  • Identifying general workplace trends

  • Developing benchmarking insights and reporting

This data will not identify you or your Employer.

3. Working From Home Checklist Requirements
As part of this process, you may be asked to:

  • Complete an online checklist to identify potential risks in your home working environment

  • Upload photos of your workstation, equipment, and any relevant safety concerns

Before submitting photos, you should:

  • Remove any confidential, personal, or sensitive information from view

  • Ensure no individuals (including yourself) are visible in any images

4. Nature of Information Provided
The content, including any videos, guidance, or recommendations provided within this platform:

  • Is general in nature and provided for educational purposes only

  • Does not constitute individual advice, medical advice, or a formal risk assessment

  • Should not be relied upon as a substitute for professional or workplace-specific guidance

5. Limitation of Liability
To the extent permitted by law, AHP:

  • Makes no representations or warranties regarding the accuracy, completeness, or suitability of the information provided

  • Accepts no liability for any loss, injury, or damage arising from the use of, or reliance on, this platform or its content

Any actions taken by you or your Employer based on the information provided are done at your own discretion.

6. No Obligation to Act
Neither you nor your Employer are under any obligation to implement any recommendations or guidance provided through this platform.

7. Employer Responsibility
Your Employer is responsible for:

  • Reviewing and interpreting the information submitted

  • Determining appropriate actions or interventions

  • Meeting their workplace health and safety obligations

8. Acceptance
By proceeding with and submitting this form, you confirm that you have read and understood these Terms & Conditions.
