We respect your right to privacy and confidentiality. The information you provide to Active Health Partners (AHP) is securely stored on encrypted cloud-based servers and will not be shared with third parties, without your consent.

By agreeing to the Terms & Conditions you are acknowledging that, in completing and submitting AHP’s Working From Home Checklist & WHS Training online form, all of your responses, including any uploaded photos, will be automatically sent to you and your Employer.

You will be asked to provide some personal details and information related to your employment and working from home arrangement. You will also be asked some questions about your physical and mental health. You are not obligated to provide your employer with personal or medical in-confidence information, should you not wish to do so.

You will be asked to complete a Working From Home Checklist.  This involves:

  • Completing an online checklist to help you identify potential safety hazards related to you working from home arrangement

  • Taking and submitting photos of your workspace including your workstation setup, seating and any safety issues you’ve identified that you wish to inform your employer about.

We strongly advise that you remove any confidential or personal information from your workspace prior to completing this task and ensure that there are no people in any photos, including yourself.

The video material presented in AHP’s Working From Home Checklist online form has been distributed by Active Health Partners as an information source only.
